Postanoxic electrographic status epilepticus and serum biomarkers of brain injury

چکیده

AimTo explore if electrographic status epilepticus (ESE) after cardiac arrest causes additional secondary brain injury reflected by serum levels of two novel biomarkers injury: neurofilament light chain (NfL) originating from neurons and glial fibrillary acidic protein (GFAP) cells.MethodsSimplified continuous EEG (cEEG) NfL GFAP, sampled at 24, 48 72 h arrest, were collected during the Target Temperature Management (TTM)-trial. Two statistical methods used: multivariable regresssion analysis; a matched control group patients without ESE for early predictors poor neurological outcome.Results128 had available cEEG. Twenty-six (20%) developed ESE, majority (69%) within 24 h. was an independent predictor elevated (p < 0.001) but not GFAP = 0.16) arrest. Compared to outcome, who higher 0.03) 0.04) arrest.ConclusionESE is associated with which may suggest increased neuronal compared similar initial injury. Associations reflecting are less clear. The study design cannot exclude imperfect matching or other mechanisms contributing seen in ESE.

Effects of hypothermia on brain injury induced by status epilepticus.

We investigated the neuroprotective efficacy of hypothermia after status epilepticus (SE) in immature rats. In addition, the expression levels of NMDAR1 and c-Jun were measured to establish a possible signaling mechanism for hypothermia-induced neuroprotection. Pilocarpine-treated rats were randomly divided into 2 groups: group D (diazepam) and group DH (diazepam plus hypothermia). Compared to ...

Serum prolactin during status epilepticus.

The serum concentration of prolactin is frequently increased after single epileptic seizures and has therefore been used as a method to differentiate between hysterical attacks and epileptic seizures. We determined plasma prolactin concentrations in fifteen patients with status epilepticus. Seven patients had absence status, five complex partial and three generalised tonic-clonic status epilept...

Cardiac electrographic and morphological changes following status epilepticus: Effect of clonidine

PURPOSE Status epilepticus has been increasingly associated with cardiac injury in both clinical and animal studies. Our group has previously shown that excitotoxic seizure induction results in the formation of ischaemic myocardial infarcts and loss of cardiac haemodynamic function. We hypothesised that attenuation of cardiac sympathetic/parasympathetic balance with a central presynaptic α₂ ago...

Treatment of electrographic seizures and status epilepticus in critically ill children: A single center experience

PURPOSE Electrographic seizures (ES) and electrographic status epilepticus (ESE) are common in encephalopathic children in the pediatric intensive care unit (PICU) and associated with worse short-term outcome. Survey data indicate most physicians treat ES and ESE with antiepileptic drugs (AEDs), but few data are available regarding AED usage patterns. We aimed to describe AED usage for ES and E...

Topography of generalized periodic epileptiform discharges in postanoxic nonconvulsive status epilepticus

We studied slow (≤2.5 Hz) nonevolving generalized periodic epileptiform discharges (GPEDs) in the electroencephalogram (EEG) of comatose patients after cardiac arrest (CA) in search of evidence that could assist early diagnosis of possible hypoxic nonconvulsive status epilepticus (NCSE) and its differentiation from terminal brain anoxia (BA), which can present with a similar EEG pattern. We inv...
